If you were involved in a motor vehicle crash and it was documented by a Spotswood Police Officer, you may receive a copy of the report. Reports are generally available for review within seven (7) business days following the date of the incident. Reports can be picked up at the Spotswood Police Records Bureau during normal business hours or downloaded from CRASHDOCS.

The Driver’s Privacy Protection Act (DPPA) is a federal law that limits who can access and use personal information from driver’s licenses and motor vehicle records. Under the DPPA, personal identifying information, including an individual’s date of birth, driver’s license number, social security number, home address, home telephone number, photograph, and medical or disability information, is prohibited from disclosure to the public unless one of the DPPA exceptions apply.
Personal information obtained from the Motor Vehicle Commission is utilized to complete crash reports and other police records. Therefore, crash reports that include personal identifying information prohibited from disclosure by the DPPA are not available to any person(s) other than involved parties unless an exception applies.
Uninvolved parties must complete a DPPA Permissible Uses and Certifications Form before crash reports containing personal identifying information can be released. Knowledge of what access and uses are permitted under the listed Federal Act is the responsibility of the requester.
Those improperly obtaining, disclosing, or using personal information may be held civilly and/or criminally liable. See Title 18 U.S. Code § 2721. In addition, a person who knowingly obtains or discloses personal information for any use not permitted under section 2 of P.L.1997, c.188 (C.39:2-3.4) may be held civilly and/or criminally liable pursuant to N.J.S.A. 39:2-3.5 and N.J.S.A. 39:2-3.6.
The New Jersey Self-Reporting Crash form (SR-1) is used to report a crash that was not investigated by the police, and it is the only form that can be used for this purpose.

Alternatively, the electronic versions below enable NJ citizens to fill out a form online and receive acknowledgment back within minutes. The online receipts are equivalent to the paper versions stamped by the NJDOT and can be submitted to an insurance company.
